It’s Not About Whether the Device Works — It’s About Whether the Answer Can Be Trusted

Did you know that in medical technologies, especially in vitro diagnostics, the most serious risk is not device failure, but credible wrong answers?
This distinction separates medical technologies from biotechnology in a fundamental way. In biotech, failure is often binary and biological. A drug either produces the intended biological effect or it does not. In medical technologies, harm often emerges indirectly, through decisions made on the basis of information the technology provides. The device can perform exactly as designed and still cause harm if the answer it produces is plausible, trusted, and wrong .
This difference reshapes how risk must be engineered, managed, and evidenced. It is foundational to diagnostics and other medical technologies, yet it is routinely underappreciated by early teams.
Medical technology risk is informational, not biological
Biotechnology products act on biology. Their risk is tied to exposure, dosage, toxicity, and physiological response. Failures are often observable through adverse events or lack of efficacy.
Medical technologies, by contrast, often inform action rather than perform it. Diagnostics generate results. Monitoring systems trend data. Imaging systems guide interpretation. These technologies shape downstream decisions made by clinicians, laboratories, and health systems.
In this model, harm does not require malfunction. A diagnostic system can be analytically stable, mechanically sound, and software-validated – and still introduce risk if the information it provides is incorrect, biased, or misinterpreted. A false negative delays care. A false positive triggers unnecessary intervention. A subtly biased result shifts treatment patterns across populations.
This is why the dominant hazard in diagnostics is not device breakdown, but clinical consequence of wrong information
Wrong answers are harder to detect than broken systems
Broken devices announce themselves. They stop responding, alarm, or fail basic checks. Wrong answers often do not.
Diagnostic outputs are designed to look authoritative. They are numerical, repeatable, and often consistent with expectations. When wrong, they fail silently. The system does not signal uncertainty. The clinician does not know to question the result.
Post-market analyses of failed or recalled diagnostics repeatedly show that the root cause was not a single defective component, but an interaction between assay behavior, system design, software logic, or workflow assumptions that produced plausible but incorrect results under specific conditions
This drives a higher burden on systems engineering
Because diagnostic risk emerges from interactions, medical technologies carry a higher systems-engineering burden than many early teams anticipate.
Assay chemistry, hardware, software, consumables, calibration models, and user workflow are not independent. A small change in one domain can propagate invisibly into others. Examples documented across development lifecycles include:
- Reagent stability interacting with thermal control limits
- Algorithm updates shifting result distributions near clinical cutoffs
- Consumable design changes altering sample handling variability
- Workflow assumptions breaking under real laboratory throughput
These failures rarely appear in early feasibility. They surface during integration, verification, or clinical use – when changes are slow and costly. Systems-engineering lifecycle analyses consistently show that late-stage failures trace back to missing or unstable system-level requirements defined at the front end .
Risk management must anchor to clinical consequence, not just failure modes
In biotechnology, risk analysis often centers on biological mechanisms and exposure pathways. In medical technologies, especially diagnostics, risk management must begin with clinical consequence.
The same analytical error has very different implications depending on use context. A small bias may be tolerable in trend monitoring but unacceptable in rule-out diagnostics. A false negative in screening carries different consequences than in confirmatory testing.
Failure analyses show that teams often discover too late that their performance targets and verification plans were misaligned with the clinical stakes implied by their intended use. When regulators ask why certain risks were not mitigated, the answer is rarely “we ignored them.” More often, it is that the clinical implications were never explicitly modeled early enough
Traceability matters because harm is indirect
Because medical technology harm is indirect, regulators rely heavily on traceability to establish confidence.
Traceability links intended use to performance claims, claims to requirements, requirements to verification, and verification to risk controls. This chain allows reviewers to understand not just what data exists, but why it exists and how it mitigates specific clinical risks.
Weak traceability signals that evidence generation was opportunistic rather than intentional. It suggests that results may be correct, but not necessarily sufficient to support the claims being made. This is why diagnostics face intense scrutiny of design history and verification rationale, even when the underlying technology appears sound .
Why early teams miss this distinction
Early medical technology teams are often led by strong scientists and engineers who naturally focus on technical feasibility. When a system produces stable, repeatable results, it feels “done.”
The problem is that informational risk is statistical, contextual, and clinical. A system can perform correctly and still mislead. A result can be analytically valid and clinically unsafe in specific populations or workflows.
This is why many programs appear healthy until verification or regulatory review exposes gaps that feel sudden but were present all along.
The real takeaway
Medical technologies do not fail primarily because devices break. They fail because trusted information quietly steers decisions in the wrong direction.
Teams that internalize this early design differently. They invest earlier in systems engineering, anchor risk management to clinical consequence, and treat traceability as a design tool rather than a documentation task.
Those that do not often discover – too late – that the most dangerous failures never looked like failures at all.
